The legal date will be the date that this form is submitted for processing. "I hereby hold blameless and fully release the Professional Disc Golf Association (PDGA), Quad City Disc Golf Club, the tournament directors, local and state park services, and all tournament partners/sponsors and individuals or parties associated with the QCDGC Rumble, from all liability of personal damage or injury which I may incur before, during or after my participation in this event. I accept full and sole responsibility for any damage I may cause through negligence and/or malice, including damages or injuries caused by my own throws in practice or competition, as well as damage occurring at any of the venues where event activities are conducted. I also recognize that the PDGA, event staff, and any other media outlets covering the event have the right and responsibility to record any participants via video and still photography. Such photography may be used for event publicity, public information or any other promotional use that falls within PDGA or event activities." Note - A Parent or Guardian must provide signature if player is under 18 years of age.

In order to aid in speed of play, and to keep the event from filling, we have divided the field into two pools.
POOL A: All Pro divisions and Advanced Men (MA1)
POOL B: All other amateur divisions including Advanced Masters and Grandmasters
Sign in/registration: 7:00am – 8:00am
Player Meeting: 8:15am
Tee Off: 8:30am
